About Your Visit

Please allow at least one hour for your initial appointment.

While our goal is to see patients at their scheduled appointment time, our doctors are surgeons and can occasionally be called into emergency surgery or to see a patient with a medical emergency. If an emergency arises we will do our best to notify you prior to your appointment, however that is not always possible. Your patience and understanding is greatly appreciated.

Please prepare for this by bringing:

• Health Insurance Card / Information
  
• A friend or family member to take notes / help with questions
  
• A list of all medications you are taking
  
• A list of your questions /concerns/ past medical history and surgeries
  
• A copy of your XRAYS / CT scans (not just the reports - please get the actual films)
  
• Copies of records from doctors related to your sinuses / nose / allergies / lungs.
  
• Completed FORMS from our Website - Secure Patient Access
